Coordinates required services for children with special needs and makes appropriate referrals; Coordinates and participates in the development of an individualized, goal-oriented service plan; Facilitates and participates in the development, review, and evaluation of individualized Family Service plans; Coordinates with medical and health care providers; Facilitates the development of a transition plan to preschool services if appropriate or to other available supports and services; Coordinates County responsibilities for the transportation of children in the Early Intervention and Pre-School program to needed services; Acts as departmental liaison with service providers and monitors related service contracts; Represents the County department at meetings with local agencies and schools regarding programs and services for children with special needs; Completes detailed reports and records related to the program; Other duties as assigned, including (but not limited to) participation in departmental drills and clinics as part of Public Health Emergency Preparedness and Response activities.
Other related work as assigned.
MINIMUM
Qualifications:
a.
Possession of a Bachelor's Degree in a health or human services field; OR b.
Possession of an Associate's Degree in a health or human services field AND two (2) years of experience in service coordination activities (any type of service coordination with any age group) as defined below; OR c.
Graduation from high school or possession of a high school equivalency diploma and four (4) years' experience in service coordination activities (any type of service coordination with any age group) as defined below.
NOTE:
Verifiable part-time and/or volunteer experience may be pro-rated towards meeting the full-time experience requirements.
NOTE:
Preference may be given to candidates who demonstrate knowledge and understanding in the following areas:
a.
Infants and toddlers who may be eligible for early intervention services.
b.
State and federal laws and regulations pertaining to the Early Intervention Program.
c.
Principles of family-centered services.
d.
The nature and scope of services available under the Early Intervention Program and the system of payments for services in the State.
NOTE:
Service coordination activities may be defined as person-centered planning, case management, procuring, coordinating, and monitoring of services under an individualized support plan to establish desired outcomes, determine needs and identify resources for a person who is eligible for specialized services and advocating for the person.
SPECIAL REQUIREMENTS FOR ACCEPTANCE OF APPLICATIONS:
Possession of a valid New York State Driver's License is required.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
